The Cancellation form MUST be used at the time of canceling "membership" for any recurring monthly service being charged in such manner. Client must submit the Cancellation Request Form located below on the footer section and at top right side next the members tab.

Cancellation Forms submitted ON or BEFORE the 14th day will not be billed the following month.

Cancellation Forms submitted AFTER the 14th day will be billed the following month.

Should a member decide to reactivate a membership after termination, the CURRENT membership rates in effect at the time of reactivation will be applied (membership rates are subject to change).
